Why normal blood work doesn’t always match how you feel


Most women come to me with the same story.


They’ve had their thyroid checked.

They’ve been told the numbers look normal.

But their body has been saying something different for a long time.


The fatigue that hits before noon.

The brain fog that makes everything feel harder.

The hair shedding.

The cold hands.

The weight that won’t shift.

The cycle changes that seem to happen out of nowhere.

The sense that something deeper is going on.


And here’s the part most women don’t know.


Your thyroid is not one number.

It is an entire conversation inside your body.

Hormones.

Conversions.

Nutrients.

Immune activity.

Stress physiology.

Cellular response.


TSH only captures a single sentence of that conversation.


No wonder so many women feel unheard.




Why TSH Isn’t Enough


TSH is often treated as the gold standard, but it’s only a message from your brain to your thyroid.

It does not tell you:


  1. how much hormone your thyroid is making

  2. how well you convert T4 into active T3

  3. whether your cells can actually use thyroid hormone

  4. whether inflammation or stress is slowing things down

  5. whether your immune system is attacking your thyroid


This is why so many women fall through the cracks.


TSH can be normal while everything underneath it is struggling.


And your symptoms are often the first sign something is off long before TSH changes.





What a Full Thyroid Testing Actually Shows


Complete thyroid testing gives you the full picture.

It turns vague symptoms into clear patterns.


Here’s what each marker reveals.

TSH

  • This is the signal. It tells your thyroid what to do, but it does not show how well your thyroid is doing it.


  • Most women feel best when their TSH is between 1.0 mIU/L and 2.5 mIU/L

  • If you are trying to get pregnant is needs to be under 2.5 mIU/L


  • TSH between 2.5 mIU/L and 4.0 mIU/L with symptoms usually suggests another system may be affecting how they thyroid functions.

Free T4

  • This is your storage hormone, it shows how much hormone your thyroid is releasing. Your body needs to convert this inactive hormone to the active thyroid hormone.

Free T3

  • Your active hormone. This is the hormone your cells actually use.

  • Low Free T3 can cause symptoms even when TSH and T4 look fine.

  • You may notice: fatigue, weight that won’t budge, brain fog, dry skin, constipation

Reverse T3

  • Your body’s brake pedal.

  • Reverse T3 rises when your body feels unsafe, depleted, or overwhelmed.

  • High Reverse T3 blocks T3 from doing its job.

Anti TPO & Anti TG

  • These are antibody tests that identify Hashimoto’s

  • They can be positive years before TSH changes

Helpful Add On Tests

  • These markers add depth to your thyroid story

  • Ferritin: low iron storage slows conversion of T4 to T3

  • Vitamin D: supports immune balance and hormone function

  • Cortisol: stress affects thyroid conversion, PMS, sleep, & metabolism

  • Vitamin B12: needed for energy, mood, & neurological health

  • Insulin: high insulin worsens conversion issues & weight struggles

  • Lipids: elevated LDL & triglycerides often show up in low thyroid states


How These Markers Work Together

The pattern tells the truth.

Low T4 with normal TSH: Your thyroid is slowing down before the brain notices.


Low T3 with normal T4: A conversion issue. Common with stress, inflammation, low iron, and poor sleep.


High Reverse T3 with low T3: Your body is overwhelmed and blocking hormone activity.


Normal TSH with positive antibodies: Early Hashimoto’s.



Symptoms are real even if you’re told it’s too early to treat.


Normal labs with classic symptoms

Your levels may be normal, but not optimal.

Your body is still asking for support.




When You Should Ask for a Full Thyroid Testing


  • fatigue that doesn’t match your lifestyle

  • persistent hair loss

  • weight loss resistance

  • cold hands and feet

  • brain fog

  • heavy or irregular periods

  • fertility concerns

  • anxiety with exhaustion

  • slow digestion

  • postpartum shifts



  • a family history of thyroid disease

  • autoimmunity

  • a thyroidectomy

  • miscarriage history

  • temperature issues

  • elevated cholesterol without a known cause




What Happens When Only TSH Is Checked


Women are often:

  • told everything is normal

  • told it’s just stress

  • misdiagnosed

  • placed on medication without follow up

  • left to figure things out alone


TSH only testing misses:

  • early hypothyroidism

  • early Hashimoto’s

  • conversion problems

  • Reverse T3 issues

  • nutrient deficiencies

  • stress related blocks


No wonder so many women feel dismissed.




How I Interpret These Results in Practice

I don’t isolate numbers and I don't treat you as just a number.

I look at the pattern and your lived experience.


I consider:

  • your symptoms

  • your goals

  • your nutrients

  • your digestion

  • your stress physiology

  • your sleep

  • your cycle rhythm

  • your metabolism


Your thyroid depends on your gut, liver, immune system, protein intake, and stress hormones.

When you support these systems, thyroid symptoms often begin to lift.
